To ensure the safety of our students, the Archdiocese of Chicago requires all of our volunteers to complete a five-step process before they can come into contact with the students. This process must be completed for any activity involving students, even if it is a one-time event. This includes chaperones, youth ministers, adult leaders, parents supervising field trips, lunch room supervisors, coaches, etc. Our site administrator is Letty Garcia and she will be keeping track of all of our volunteer information. The process consists of the following:
Criminal Background Check – completed online through the eApps database (the online form looks similar to job application - instructions below)
VIRTUS training sessions (3 hour training workshop) – online registration is required – see below
Annual completion of a CANTS Department of Children & Family Services (DCFS) form (please return to Letty Garcia)
Completion of the Archdiocese of Chicago’s Code of Conduct Form (please return to Letty Garcia)
DCFS Mandated Reporter Training form AND certificate (online training) – directions below
Instructions for completing the online Criminal Background Check:
Visit the Archdiocese of Chicago website.
At the top, select “First time users are asked to click here” to create your username and password.
Your location is St. Anastasia School and the access code is PROTECTION.
Follow the steps to complete the process and be sure to click SUBMIT at the end.
Instructions for registering for VIRTUS training:
VIRTUS sessions are held year-round at various locations and are free of charge.
To check for upcoming sessions and register online, visit VIRTUS Online.
Once you have completed VIRTUS training, please let Letty Garcia know.
If you have completed your training through another Archdiocese, please contact the VIRTUS Consumer Line (1-888-847-8870) to have your record transferred to the Archdiocese of Chicago.
Be sure to read and respond to your VIRTUS TRAINING BULLETINS you will receive via email to remain active by the Archdiocese.

View the video and then print your form and certificate and give a copy to Mrs. Garcia.
CANTS and Code of Conduct Forms:

Print, complete, and return copies to Letty Garcia.
